Medical and Personal Information

This information is only collected in relation to an assessment or intervention and not via this website. These records will be kept securely in line with GDPR (General Data Protection Regulations) and will not be shared with any third parties. All information will be stored on a password protected computer, using Microsoft One Drive.

What Personal Information Do I Hold?

  • You or your child’s past and current medical; personal details such as age, address, telephone & medical practitioner
  • Information about treatment or assessment, I have provided, and its cost
  • Notes of conversations or incidents that might occur for which a record needs to be kept
  • Records of consent to treatment
  • Any correspondence relating to you with other health care professionals, for example in the hospital or community services. This also includes school / learning environments.

Why Do I Hold This Information?

To keep accurate personal data about patients to provide you with safe & appropriate Occupational Therapy assessment and intervention.  

Retaining Information

I am required to retain your records, while you are involved in ongoing intervention and after you cease this, for at least 11 years or until age 25, whichever is longer.

Security

Your information is held on my computer and in a secure manual filing system. The information is only accessible to me.

Your information will be kept confidential unless you have given explicit consent for this to be shared with others, such as other healthcare professions, social workers or schools. I will not disclose your information to any third party without your permission unless there are exceptional circumstances (i.e. life or death situations). This is where the law requires information to be passed on and / or in accordance with the information sharing principle following Dame Fiona Caldicott’s information sharing review (Information to share or not to share) where “The duty to share information can be as important as the duty to protect patient confidentiality.” This means that health and social care professionals should have the confidence to share information in the best interests of their patients within the framework set out by the Caldicott principles. They should be supported by the policies of their employers, regulators and professional bodies.
